Intrauterine insemination (IUI) is an artificial insemination procedure and a technique to resolve infertility. In this procedure, the treated sperm are placed directly in the uterus when the ovaries release one or more eggs for fertilization.

 The outcome of IUI is for the sperm to swim into the fallopian tube and fertilize a waiting egg, resulting in pregnancy. The use of IUI depends upon the reasons for infertility and is done with the regular menstrual cycle or with fertility medicine in certain situations.

Dr Mohit Saraogi, founder of one of the best IVF centre in Mumbai and a leading IVF specialist states, 'I recommend going ahead with donor sperm and intrauterine insemination (IUI) in case there are no sperm samples available of the male partner. Also, a well-selected sperm donor implies a reduced risk of passing on a genetic disorder or disease likely to be inherited.

He elaborates on how to choose the sperm donor. First, it is necessary to know the medical history of the donor and his family, especially regarding hereditary diseases, genetic conditions, HIV, and infectious and communicable diseases. Additional screening is required if the patient wishes to have children with specific complexion, eye colour, height etc. They can always screen the donor's photos to get a fair idea of the same.

 Some patients can also opt for known donors, such as friends or family members, while others prefer anonymous donors, which is entirely their decision. If patients are keen to bear more than one child in the future, that too, from the same donor, then storing additional sperm is also a good idea.

Scheduling the IUI procedure is very important. The IVF specialists check s the patient on the first day of the menstrual cycle, and on the following days, the doctor measures the hormonal levels with a blood test. Then, the doctor can prescribe fertility medicine along with regular follow-ups. If required, an injection of human chorionic gonadotropin (HCG) or medications is given for ovulation of one or more as per the cycle. Upon detection of ovulation, the IVF specialist prepares the sperm and proceeds to perform the IUI procedure.  

The IUI procedure is a simple one. The chosen sperm specimen is treated and inserted through the cervix within the uterus using a catheter. The IVF specialist injects the sperm into the uterine cavity. The patient can continue to lie for some time and resume daily activities. The patient can experience slight spotting for a couple of days after the procedure.

 It takes nearly two weeks to detect whether or not the IUI is successful. Most doctors recommend visiting the clinic for pregnancy detection rather than relying on pregnancy tests done at home.

There are chances that patients may need to undergo 3 – 4 IUI cycles before they get pregnant, while some of them get pregnant in the first cycle. Here, the patient's age is an important factor for success in IUI treatment.

For patients below 35 years, it is advisable to undergo at least 3 – 6 cycles before proceeding with the IVF treatment. Patients above 35 years need to consult an experienced IVF specialist to decide the best path forward.
